Reasonable Suspicion Report
The Reasonable Suspicion Report is to be completed prior to sending an employee in for drug or alcohol testing due to reasonable suspicion.
Available as a downloadable DOC file, this report provides specific guidelines and instruction for handling violations of drug and alcohol policy and procedure. In order to remain compliant, two members of the management team must complete the report by outlining specific firsthand observations or conversations with the employee in question.
